Recovery following a significant injury can be complicated! We provide case management services to simplify the process and assist you on your recovery journey. We have worked with NIISQ, Icare, WorkCover and the NDIS.

What does a case manager do?

Case managers work with the participant to identify their goals and support needs. They also work with service providers, staff of insurers or funding bodies, your family and with service providers to coordinate the services you need to reach your full potential.

A case manager is someone who will:

help you understand your injuries and how they may change your life

understand your funding and the options available to you

N

assist you to identify your goals, and barriers that may stop you from achieving your goals

N

provide guidance on which services will help you

N

build a plan with you to achieve your goals

N

coordinate and monitor your services, directing therapy towards your goals

N

progress your rehabilitation and monitor your outcomes

N

coordinate your care and/or support needs

N

where applicable liaise with your funding source
